Usually straight from the manufacturer, but there are a host of permits and steps to take.
You have to apply for a BB-IND number from the Center for Biologics Evaluation and Research (CBER), fill out a form 1572, apply to the U.S. Dept. of Agriculture for a permit to import antivenom, clear all antivenom importations through U.S. Customs, etc. etc. etc.
If you are really serious about importing antivenom for exotic species, try to get a copy of the Antivenom Index. It explains how to get antivenom, step by step. There's no easy way to get it.
