You would almost certainly be asked to sign such a waiver for exotic antivenom anyway. I would be surprised if they didn't have a notary in the building if it's necessary for a waiver.
But yeah, having one on hand already notarized might not be a bad idea in case you're unconscious...right next to your "no fasciotomy without a fourth opinion" letter,

Perhaps someone can put those Waiver Letters here on the site in PDF form, so that site members can add them to protocol kits ?

Well, I was just looking through some old posts, and found this topic.
I PERSONALLY WOULD BE VERY PLEASED IF PERHAPS AL OR SOMEONE ELSE COULD PROVIDE THAT WAIVER IN PDF OR .TXT FILE FORMAT TO THIS SITE! Another greatr sugestion by John! LOL...
=( Never really though about it much, but that would be VERY useful, and would be at a loss to right up some sort of appropriate document as such myself.
I know very little about toxinology and the treatment given by doctors treating envenomation, so my bite protocols leave much to be desired, and I often question the usefulness of my personal information..
A copy of such a waiver would make my protocols at least a little bit more prepared for what I hope to never find myself in the middle of one day!
