I recently bought a Mark O'Shea 40" signature series snake hook and love it. I have to ask though has anybody had any problems with this hook? I have heard that in some cases the head of the hook is wrong. Mine doesn't seem to have this problem. Are there any other problems with this hook I should be aware of? Has anybody ever had one of these break on them?

No, mine has never had any problems. I've seen some of the damaged stuff that people have sent back to Midwest, and most of it looks like people used it to try to demolish a house, which obviously isn't what those hooks are intended for.
IF you use that hook "to handle snakes" you will never have a problem with it. If you use it as a pry-bar to lift fallen concrete walls, well then - yes, you will probably have an issue or two.
Midwest will fix anything that they deem has genuinely failed under normal use conditions, and they use honest judgement to determine that. I know. I've been there.
