Longterm effects of snakebite survey

The Florida Snakebite Institute and Kentucky Reptile Zoo are collaborating on a research study in to the Long term effects of snakebites. Immediate effects of snakebites are well known, but little research has been done on the lasting effects months to years later. We are currently doing a subjective survey of individuals who have sustained snakebites looking at any lasting effects from those bites. The survey is a simple 3 pages of questions, and no personal information is required except age at time of bite and current age.
If interested in assisting us and completing the survey, please email me at traumajunkiejoe@Juno.com providing me with your email, and I will send the survey to you. We are hoping to get data back quickly in order to present the results at the 2016 Venom Week Conference. Thanks for your assistance.
