Pain was probably caused by the shrew toxins that are mutated versions of the kallikrein enzymes called Blarina toxins. This same toxin type is convergently found in reptile and other venoms and is often responsible for extreme pain and swelling, such as the fun and games people go through following Heloderma envenomations.
