Shipping Question from Europe to US What's needed?

First off, please forgive the lack of information on my behalf.
The situation is that I have located an European breeder who is willing to ship to the US, yet requires the customer to set up all paperwork.. Alright, so this is my attempt at being educated on what's needed.
The species is not listed under CITES so that much of it is good news. What else do I need other than the USFW import/export permit?
